Immunization Requirements for Attendance in U.S.D. #475

Pre-K – Grade 5

**Changes for the 2009-2010 school year are underlined**

 

Kansas law requires that students must show proof of having received all age appropriate immunizations to attend school.  Immunization records from a licensed doctor or health department will be accepted

for this requirement.  (Age is student’s age on September 1, 2009)

Immunizations Required for Students Kindergarten (age 5) – Grade 5

 

DPT, DTaP and/or DT………….5 doses (4 are adequate if dose 4 is after the 4th Birthday)

Ψ  TdaP Booster at age 11

Students who will be 12 years of age on September 1, 2009 will be required to have documentation of Tdap vaccination if it has been more than 2 years since the previous dose of Td/DTaP/DT.

 

 Polio………….………………..…4 doses (3 are adequate if dose 3 is after the 4th Birthday)

 

MMR………….…………….……2 doses (Dose 1 must be after the 1st birthday)

 

Varicella (Chickenpox)………….1 dose required, unless history of chickenpox                    

                                                         disease is verified by a licensed physician

Ψ  Kindergarten students are required to have 2 doses of Varicella, unless history of chickenpox is verified by a licensed physician.  (Dose 1 must be after the 1st birthday)

 

Hepatitis B………………………..3 doses required

 

For the 2010-2011 School Year:

**Additional grades may be required to have 2 doses of Varicella.

 

Immunizations Required for School Attendance for Pre-K (ages 3-4)

           

DPT, DTaP and/or DT…….……..4 doses

Polio…………………………….…3 doses

MMR……………………………...1 dose (after 1st birthday)

Hepatitis B series (3 doses) and Varicella (1 dose after 1st birthday)

AND age appropriate immunizations for Hib, Hepatitis A and Pneumococcal Conjugate PC7

 

A school nurse will review immunization records for these and other requirements.
